PROCESSING FEE

Range of fees for requesting a medical report are as follows:
* Non-Specialist report
RM55 -RM230
* Specialist report
RM115 – RM570

Payment can be made using any of the following methods:
* In-person at the Admission & Discharge Counter (Lobby, Level 1) by using cash (Ringgit Malaysia only)
* Debit/Credit Card
* Banker’s cheque
* Online transfer
* E-Wallet

When your medical report is ready, you have the option to:
* Have it sent to you via courier
* Collect it at our Front Office reception counter during business hours.
* Receive a soft copy via email

PROCESSING TIME

* Standard medical reports or insurance claims will be available for collection within fourteen (14) business days after payment.
* In some cases, requests may take longer to process if there are incomplete information provided.

Who can request for medical reports?

  • Patients who are 18 years or older and in good physical and mental health – no additional documentation required.
  • Patients who are under 18, or unable to provide consent due to mental incapacity – photocopy of relevant identification or legal documents from a parent or legal representative is required.
  • For requests on behalf of deceased patients – proof of relationship or legal representation, death certificate, marriage certificate, Letter of Administration, or grant of probate is required, and nominee letter from the insurance company is needed for claim processing.

What are the documents required for medical reports application?

  • Download and fill up consent form and declaration & authorisation letter
  • Formal application, letter, query Letter, Insurance claim form, Lawyer letter, EPF form, PERKESO form.
  • If you are applying for the hospital to fill out insurance forms, questionnaires, endorsements, PERKESO, or other related paperwork, ensure that all the related information (patient’s name, NRIC, date of admission etc) are accurately provided

How do I request for a medical report?

  • Email completed form to Customer Service
    You will receive an acknowledgement email within two (2) business days.
  • You may also submit your request in-person at the Front Desk counter (Lobby, Level 1, Northern Heart Hospital) during regular business operational hours.
  • Your request will then be acknowledged by The Medical Records Officer, whereby you will also be informed of any applicable administrative fees.

How much is the medical reports fee?

The range of fees for requesting a medical report are as follows:

  • Non-Specialist report     |     RM55 – RM230
  • Specialist report               |     RM115 – RM570

Please be aware that this schedule is subject to change periodically. Any additional charges applicable to the report will be communicated to you accordingly.

How do I make payment for my medical report?

  • Payment can be made using any of the following methods:

  • Note that personal cheque payments will require clearance before the report and receipt are issued.
  • It is also important to note that settlement of any outstanding balances along with the medical report fee is required to prevent processing delays.
  • Upon, payment, please email proof of payment to Medical Record
  • Please include Patient MRN as a reference to expedite processing and avoid delays.

When will I receive my medical report?

  • Standard medical reports or insurance claims will be available for collection within fourteen (14) business days after payment, once all necessary documents and payment have been received.
  • In some cases, requests may take longer to process if:
    • Information provided are incomplete.
    • Doctor going on leave, patient has upcoming appointments, or patient is still hospitalised.
    • Multiple reports are requested from different doctors.

How will I receive my medical report?

  • Our Medical Records Department personnel will be in touch with you once your report(s) is ready.
  • You have the option to:
    • Have it sent to you via courier to the mailing address specified in the Application Form. Courier charges may apply.
    • Collect it from our Front Desk counter (Lobby, Level 1, Northern Heart Hospital) during regular business operational hours. Please bring proof of identity (NRIC/Passport) or if collecting on behalf of the patient, please provide an authorisation letter signed by the patient.
    • Receive an encrypted (with password) soft copy via email to the address provided in the Application Form.
