General Consent for Treatment and Evaluation

Voluntarily consent to care and treatments including but not limited to physical examinations, diagnostic tests, medical procedures and medications by the Specialist Consultant and clinical staff of Northern Heart Hospital (NHH) as deemed necessary or advisable in their professional judgment.

Please note that you would be required to sign a specific informed consent form for specialised investigation and invasive procedures and general surgery.

Patient Right

  • Right to Refuse Treatment :
    I understand that I have the right to make informed decision regarding medical management and that I could ask my healthcare provider for further clarification and explain on matters related to my treatment. This includes, the right to refuse any treatment.
  • Right to Access and Update Patient Information :
    Northern Heart Hospital (NHH) respects the rights of our patient to access their personal and health information through medical reports and insurance claims. Patient also have the rights to make corrections to their personal information and the right to instruct the hospital not to disclose their condition to anyone including family members.

What Information That We Process?

The hospital processes personal data provided by individuals in connection with healthcare services, including information such as name, address, contact details, and other relevant documents. If an individual provides personal data about another person, they must confirm that the person has appointed them to act on their behalf.

The hospital may request assistance in obtaining consent from individuals whose personal data is provided by the individual, and the individual is responsible for indemnifying the hospital in the event of any loss or damage resulting from a failure to comply with this requirement.
